Once you arrived at EVOLVE, you will find us on the third door down the hallway to the right. Our door will be open at the time of your appointment, please grab a seat and our therapist will be with you shortly.
You are no longer required to complete the screening questionnaire, prior to every appointment. Seasonal illnesses; If you are unwell with any cold, flu, or experiencing any contagious gastrointestinal symptoms, or if someone in your family is sick, we recommend staying at home until you feel better.
Registered Massage Therapist/Service Provider retains the right to refuse service and cancel your appointment based on Registered Massage Therapist/Service Provider’s professional opinion (i.e., client arrived for service appointment – not feeling well or intoxicated).
We understand that emergency situations do come up; if you are unable to make your appointment, please contact us at least 24 hours in advance.
During these unprecedented circumstances, we ask that you are on the side of caution when it comes to deciding whether or not to cancel an appointment due to illness. We will not be charging a fee for any cancellations up to four hours before the scheduled start time of the appointment. No-shows or cancellations within four hours of the appointment will be subject to a fee (the full cost of the missed appointment).
